I verify that my child has been checked by a licensed physician and is able to participate in the Alabama Tennis Academy. I agree to allow the Alabama Tennis Academy to seek medical help in the case of an emergency if necessary. I also agree to assume all medical costs in the event of an injury. I also will allow for the disclosure of medical information to my insurance company for the purpose of a claim.

I hereby release the Alabama Tennis Academy and its employees and agents from any liability for damages to or loss of personal property, sickness, and injury from whatever source, legal entanglement, death, loss of money, etc. for which the academy is not culpable, which might occur while participating in a sports camp.

Please return the completed application with a deposit of $100.00. All checks and money orders should be made payable to the Alabama Tennis Academy. The deposit is non-refundable and is due June 25.
